• At Concierge Senior Care, we understand that finding the right senior living community for your loved one can be a difficult process. That’s why we offer senior placement services that include in-person/phone consultations and guided tours of senior living communities. Our experienced team of senior placement specialists will work with you to ensure that your loved one is placed in a safe and supportive environment.

• Assist with medication management.

• Provide 1:1 discussion of current medications, side effects, adherence and physician directions.

• Promotion of regular blood pressure monitoring at home and sharing monitor readings with physician.

• Of course, we prefer to be brought in before there’s a crisis. But if one occurs—a fall, a hospitalization, a stroke, a wandering incident—we can jump in and handle the medical issues. We can also support you, and your relatives if changes in living situation are required.
